Form rekam medis8/15/2023 ![]() ![]() ![]() Based on the study's findings, it can be said that physical, chemical, and biological variables cause damage to inpatients' medical records, with the forms of damage such as being torn, loose, and yellow to brown in color. Medical record covers that are ripped, disconnected, and yellow to brown in color as a result of mildew and dust are three examples of medical record degradation caused by these three variables. The study's findings reveal the physical, chemical, and biological factors such as a cramped space and fully stocked medical record storage shelves, roof leaks that encourage mold growth, a humid environment, and the presence of rats, and cats that wander into the medical record storage room that lead to damage to inpatients' medical records. The research sample for this study consisted of 300 inpatient medical records, 1 head of the Medical Record Installation, 7 filling officers, and a qualitative-descriptive research methodology. The aim of this study was to identify the physical, chemical, and biological sources and forms of damage to medical records. Keywords : Incompleteness inpatien Medical Resume.Ĭopyright (c) 2021 Jurnal Ilmiah Kesehatan Media Husada Conclusion: to overcome the problem usually requires more optimal socialization and collaboration between doctors and medical recorders, to further improve health services regarding the completeness of filling out medical record documents in order to improve the quality of health services, and the head of medical records needs to hold meetings or directives to the unit concerned so that the resume is filled out. The problem that usually arises is that there are still many incomplete components of a medical resume. And the highest percentage of completeness is no medical record (76%). The highest percentage of incompleteness is the doctor's signature (52%). Results: Based on the results of the study, the incomplete medical resume form for inpatients in 2021 with a sample of 50 medical resume forms, the average percentage of medical resume completeness was 66.7%, while the incomplete medical resume was 33.3% of the 50 samples of medical resume forms. The data analysis used is descriptive data analysis. Method: The sampling technique used is the total sample. In this study there were 4 medical recorder and 50 medical resume form. This research is a descriptive research with a qualitative approach. Incomplete filling of medical resumes can be caused by negligence of officers or lack of understanding of the importance of completing patient data. Preliminary: A medical resume is a summary of medical service activities provided by health workers, especially doctors during the treatment period until the patient is discharged either in good health or in death. ![]()
